Hemorrhoids(Piles)- Hemorrhoids (also known as piles) are swollen and inflamed veins in the rectum or anus. Typical symptoms are pain, itching and bleeding around the anal area.

Generally Piles are of 4 types: 

External piles- are present on the skin around the anus and can be seen or felt from the outside. The main symptoms of external piles include painful, swelling and hard lumps around the anus. If these hemorrhoids get irritated, they may cause itching or bleeding.

Internal piles- lie deep inside the rectum and cannot be felt or seen from the outside. They are mostly painless since there are no pain receptors in the region where they tend to appear. The only symptom of internal piles is bleeding through the anus while straining during a motion of the stools.

Protruding or prolapsed piles- are hemorrhoids that get pushed out towards the anus when a strain is applied to pass stools. They look like pink, moist pads of skin. Prolapsed or protruding hemorrhoids tend to hurt since the anus area is dense with pain-sensitive nerves. The typical symptoms of hemorrhoids are mucus discharge, itching, and pain.

Blind Piles- are those hemorrhoids that do not bleed. They feel like protruding masses, usually detectable after passing stools, at the opening of the anus.


Piles can also be classified into grades/degree , according to their severity.
  • 1st Degree piles : Remain in the rectum
  • 2nd Degree piles: Prolapse through the anus on defecation but spontaneously reduce
  • 3rd Degree piles: Prolapse through the anus on defecation but require digital reduction
  • 4th Degree piles: Remain persistently prolapsed
Causes: 
  • Straining during bowel movements
  • Sitting for long periods of time on the toilet
  • Having chronic diarrhea or constipation
  • Being obese
  • Being pregnant
  • Having anal intercourse
  • Eating a low-fiber diet
  • Regular heavy lifting
Signs and symptoms:
  • Pain or discomfort, especially when sitting
  • Pain during bowel movements
  • Itching or irritation around the anal region
  • Bright red blood on your stools, toilet paper or in the toilet bowl
  • Swelling around the anus
  • One or more lumps near the anus, which might be tender or painful.
Hemorrhoids Diagnosis
  • Physical exam. Your doctor will look at your anus and rectum to check for lumps, swelling, irritation, or other problems.
  • Digital rectal exam. Your doctor will put on gloves, apply lubrication, and insert a finger into your rectum to check muscle tone and feel for tenderness, lumps, or other problems. To diagnose internal hemorrhoids or rule out other conditions, you might need a more thorough test, including:
  • Anoscopy. Your doctor uses a short plastic tube called an anoscope to look into your anal canal.
  • Sigmoidoscopy. Your doctor looks into your lower colon with a flexible lighted tube called a sigmoidoscope. They can also use the tube to take a bit of tissue for tests.
  • Colonoscopy. Your doctor looks at all of your large intestine with a long, flexible tube called a colonoscope. They can also take tissue samples or treat other problems they find.
Prevention
  • Keeping your stools soft is the best way to prevent hemorrhoids from occurring. 
  • Eat high fibre foods
  • Drink plenty of fluids
  • Consider using fibre supplements
  • Avoid straining when on the toilet
  • Go to the toilet as soon as you feel the urge
  • Get plenty of exercise
  • Lose weight if you are overweight
  • Avoid sitting for long periods
  • Avoid taking medication that can cause constipation, eg. codeine based painkillers.
Complications
  • Complications of hemorrhoids are rare but include:
  • Anemia. Rarely, chronic blood loss from hemorrhoids may cause anemia, in which you don't have enough healthy red blood cells to carry oxygen to your cells.
  • Strangulated hemorrhoid. If the blood supply to an internal hemorrhoid is cut off, the hemorrhoid may be "strangulated," which can cause extreme pain.
  • Blood clot. Occasionally, a clot can form in a hemorrhoid (thrombosed hemorrhoid). Although not dangerous, it can be extremely painful and sometimes needs to be lanced and drained.

Chronic use of laxatives: Laxatives or purgatives are habit-forming. Using laxatives for a long time and then stopping their use may cause more constipation. Constipation may further lead to fissure.

Underlying other diseases: Fissure may be a manifestation of an underlying disease, such as

Symptoms
  • Pain, sometimes severe, during bowel movements
  • Pain after bowel movements that can last up to several hours
  • Bright red blood on the stool or toilet paper after a bowel movement
  • A visible crack in the skin around the anus
  • A small lump or skin tag on the skin near the anal fissure
Complications
  • Failure to heal. An anal fissure that fails to heal within eight weeks is considered chronic and may need further treatment.
  • Recurrence. Once you've experienced an anal fissure, you are prone to having another one.
  • A tear that extends to surrounding muscles. An anal fissure may extend into the ring of muscle that holds your anus closed (internal anal sphincter), making it more difficult for your anal fissure to heal. An unhealed fissure can trigger a cycle of discomfort that may require medications or surgery to reduce the pain and to repair or remove the fissure.
Prevention
  • You may be able to prevent an anal fissure by taking measures to prevent constipation or diarrhea. 
  • Eat high-fiber foods,

Diagnosis:
  • The diagnosis of a fissure in ano is done by the evaluation of medical history and clinical examination. 
  •  The doctor may advise sigmoidoscopy or colonoscopy in some cases to rule out other disease conditions affecting the rectum and anal canal.
 Anoscopy. An anoscope is a tubular device inserted into the anus to help your doctor visualize the rectum and anus.

Flexible sigmoidoscopy. Your doctor will insert a thin, flexible tube with a tiny video into the bottom portion of your colon. This test may be done if you're younger than 50 and have no risk factors for intestinal diseases or colon cancer.

Colonoscopy. Your doctor will insert a flexible tube into your rectum to inspect the entire colon. This test may be done if you are older than age 50 or you have risk factors for colon cancer, signs of other conditions, or other symptoms such as abdominal pain or diarrhea.

Types of Rectal Prolapse
There are three types of prolapse:

External prolapse: The entire rectum sticks out of your anus.

Partial prolapse -  the lining of the rectum slides out of place and usually sticks out of the anus. This can happen when straining to have a bowel movement. Partial prolapse is most common in children younger than 2 years.

Internal prolapse: The rectum has started to drop but is not yet sticking out of your anus.

Causes of  rectal prolapse 
  • Chronic (long-term) constipation or chronic diarrhea
  • Long-term history of straining during bowel movements
  • Older age: Muscles and ligaments in the rectum and anus naturally weaken with age. Other nearby structures in the pelvis area also loosen with age, which adds to the general weakness in that area of the body.
  • Weakening of the anal sphincter: This is the specific muscle that controls the release of stool from the rectum.
Earlier injury to the anal or pelvic areas

Damage to nerves: If the nerves that control the ability of the rectum and anus muscles to contract (shrink) are damaged, rectal prolapse can result. Nerve damage can be caused by pregnancy, difficult vaginal childbirth, anal sphincter paralysis, spinal injury, back injury/back surgery and/or other surgeries of the pelvic area.

Other diseases, conditions and infections: Rectal prolapse can be a consequence of
  • diabetes, 
  • cystic fibrosis,
  • chronic obstructive pulmonary disease,
  • hysterectomy,
and infections in the intestines caused by parasites – such as pinworms and whipworms – and diseases resulting from poor nutrition or from difficulty digesting foods.

Symptoms of rectal prolapse: 
  • The symptoms of a prolapsed rectum are similar to those of hemorrhoids.
  • Feeling of a bulge or the appearance of reddish-colored mass that extends outside the anus. 
Other signs are:
  • Anal pain, itching, irritation, and bleeding.
  • Bright red tissue that protrudes from the anus.
  • Feeling off full bowels and urgent need to have a bowel movement.
  • Feeling of not being able to empty the bowels completely.
  • Inability to control bowel movements that becomes worse over time.
  • Passage of many very small stools.
Diagnosis:

Anal electromyography (EMG): This test determines if nerve damage is the reason why the anal sphincters are not working properly. It also examines the coordination between the rectum and anal muscles.

Anal manometry: This test studies the strength of the anal sphincter muscles. A short, thin tube, inserted up into the anus and rectum, is used to measure the sphincter tightness.

Anal ultrasound: This test helps evaluate the shape and structure of the anal sphincter muscles and surrounding tissue. In this test, a small probe is inserted up into the anus and rectum to take images of the sphincters. 

Colonoscopy: This is an exam of the colon or large bowel. A flexible tube with a camera is passed through the anus upwards to where the large intestine joins the small intestine. This helps provide visual clues as to the source of the problem.

Rectal Prolapse Prevention
  • To prevent rectal prolapse, try not to strain when you poop. Try these tips to ease or prevent constipation that leads to straining:
  • Get more fiber in your diet. Aim for at least five servings of fruits and veggies each day.
  • Drink 6 to 8 glasses of water a day.
  • Get regular exercise.
  • Keep your weight at a healthy level or lose weight if you need to.
  • If you have constipation often, talk to your doctor. They may direct you to take a stool softener or laxative.
  • Avoid heavy lifting, as this could put pressure on your bowel muscles.
Rectal Prolapse Complications
