Appendicular lump

An appendicular lump is one of the outcomes of acute appendicitis usually from third day of inflammation, formed by inflamed appendix surrounded by greater omentum, bowel loops including edematous cecal wall and ileum, which can be felt as tender mass in right iliac fossa.

Causes of Appendicular mass:
Lump formation after acute appendicitis is as a result of walled off perforation of appendix.
 Lump can be an inflammatory mass consisting of inflamed appendix, adjacent viscera, and greater omentum or a pus containing appendiceal mass.

Differential diagnosis of Appendicular mass: 
Appendicular abscess
Carcinoma cecum
Undescended testis
Tubo-ovarian mass
Ectopic kidney etc.

Diagnosis:
The patients were diagnosed by physical examination,
Computed tomography (CT),
Ultrasound.

Homoeopathic medicines: 
Pyrogen-
  • Complaints of septic origin.
  • Cutting pain in abdomen with bloating.
  • Tenesmus of both bladder and rectum.
  • Parts lay on feels sore and bruised.
  • Coffee ground offensive vomiting with obstructed bowels.
  • Constipation with complete inertia.
  • Horribly offensive stools.
  • Pulse out of proportion to temperature.
Hepar sulphur-
  • Splinter like pains with hyper sensitiveness.
  • Fainting from pains.
  • Abdomen greatly distended and tender.
  • Burning and heaviness in stomach after meals.
  • Pains aggravated by lying on painful side.
  • Stitching in liver region when moving and turning.
  • Distention of abdomen compels the patient to loosen the clothing.
Echinacea angustifolia-
  • Conditions of septic origin
  • Special affinity on vermiform appendix
  • Useful remedy in acute appendicitis
  • Its tincture can be given as a supporting drug to prevent septic appendicitis
Arsenicum album-
  • Great exhaustion after the slightest exertion.
  • Burning pains. Unquenchable thirst. Burning relieved by heat.
  • Great anguish and restlessness. Changes place continually.
  • Fears, of death, of being left alone. Great fear, with cold sweat. Thinks it useless to take medicine.
  • Abdomen swollen and painful. Pain as from a wound in abdomen on coughing.
Lachesis-
  • Abdomen tympanitic, sensitive, painful.
  • Constipated, offensive stool.
  • Hot perspiration, bluish, purplish appearance.
  • Patient sleeps into an aggravation.
  • Very important during the climacteric and for patients of a melancholic disposition.
Sulphur-
  • Should be used as a intercurrent remedy to complete the cure.
Iris tenax-
  • Well known remedy for appendicitis; pain in the ileo-caecal region, extreme tenderness on pressure.
