Acute abdomen:

The ‘acute abdomen’ is defined as a sudden onset of severe abdominal pain developing over a short time period. It has a large number of possible causes and so a structured approach is required.

Acute abdomen refers to sudden, severe abdominal pain that is considered a medical emergency, requiring immediate diagnosis and often urgent surgical intervention. People of both genders and all ages are at risk for acute abdomen because it can have such varied causes.

Natural history of frequent causes of an acute abdomen

  9. Nausea and/or vomiting
  10. Signs of shock
  11. Low blood pressure

Acute appendicitis

Acute appendicitis is a severe and sudden case of appendicitis. The symptoms tend to develop quickly over the course of one to two days . It requires immediate medical treatment. If left untreated, it can cause your appendix to rupture. This can be a serious and even fatal complication.

Risk factors of acute appendicitis:
  • Infection, possibly stomach infection that has traveled to the site of appendix.
  • Obstruction such as a hard piece of stool getting trapped in the appendix leading to infection of the appendix.
  • Extreme of age.
  • Previous abdominal surgery.
Symptom of acute appendicitis:
  • Sudden pain that begins on the right side of the lower abdomen.
  • Sudden pain that begins around your navel and often shifts to your lower right abdomen.
  • Pain that worsens if you cough, walk or make other jarring movements.
  • Nausea and vomiting.
  • Loss of appetite.
  • Low-grade fever that may worsen as the illness progresses.
Complication of acute appendicitis:
  1. Appendicular mass
  2. Appendicular abscess
  3. Recurrent appendicitis
  4. Perforation--> peritonitis
Investigation of acute appendicitis:
  1. CBC   ➧leucocytosis
  2. Urine analysis
  3. Serum creatinine and electrolytes
  4. X ray erect abdomen in diffuse peritonitis
  5. Usg abdomen
  6. CT abdomen
  7. Urine pregnancy test
Homoeopathic medicine for acute appendicitis:
  • Belladonna
  • Plumbum metallicum
  • Bryonia alba
  • Magnesia phosphorica
Acute intestinal obstruction

Acute intestinal obstruction is one of the most common surgical emergencies. Etiology of acute intestinal obstruction varies among different countries and there has also been a change over decades.

Bowel obstruction, also known as intestinal obstruction, is a mechanical or functional obstruction of the intestines which prevents the normal movement of the products of digestion. Either the small bowel or large bowel may be affected. Signs and symptoms include abdominal pain, vomiting, bloating and not passing gas.

Causes of Small bowel or Intestinal obstruction
  • Colon cancer
  • Tumors in the small intestine
  • Inflammatory bowel diseases, such as Crohn’s disease
  • Twisting of the intestine (volvulus)
  • Telescoping of the intestine
  • Hernias
  • Abdominal surgery
  • Pelvic surgery
  • Infection
  • Parkinson’s disease
  • Diverticulitis
  • RISK FACTORS OF SMALL BOWEL OBSTRUCTION
  • Abdominal or pelvic surgery
  • Crohn’s disease
  • Cancer within your abdomen
Symptoms of Small Bowel obstruction
  1. Crampy abdominal pain
  2. Abdominal distention
  3. Nausea
  4. Bloating
  5. Vomiting
  6. Diarrhea
  7. Constipation
  8. Inability to have a bowel movement or pass gas
Complications :
  1. Death of the affected intestine
  2. Infection in the abdominal cavity
  3. Intestinal perforation
  4. Peritonitis due to perforation
  5. Intraabdominal abscess
  6. Dehydration
  7. Electrolyte disturbance
  8. Death

Acute epididymo -orchitis

Acute epididymo-orchitis is a clinical syndrome consisting of pain, swelling and inflammation of the epididymis, with or without inflammation of the testes.

Epididymo-orchitis is an inflammation of the epididymis and/or testicle (testis). It is usually due to infection, most commonly from a urine infection or a sexually transmitted infection. A course of antibiotic medicine will usually clear the infection. Full recovery is usual. Complications are uncommon.

Causes of Acute epididymo-orchitis
  1. A complication from a urine infection
  2. Sexually transmitted infection
  3. The mumps virus
  4. An operation to the prostate or urethra
  5. Medication
  6. Uncommon causes (TB, AIDS)
Epididymo-orchitis symptoms
  1. Heavy sensation in the testicle area
  2. Painful scrotal swelling
  3. Fever
  4. Chills
  5. Testicle pain gets worse with pressure
  6. Lump in the testicle
Complications of Acute epididymo-orchitis:
  1. Pus formation: A collection of pus due to infection (an abscess) occasionally develops in the scrotum. This may need a small operation to drain the pus.
  2. Infertility: Reduced fertility in the affected testicle (testis), especially in cases caused by the mumps virus.
  3. Chronic inflammation: An ongoing (chronic) inflammation occasionally develops.
  4. Gangrene: Rarely, serious damage to the testicle may occur and result in dead tissue (gangrene) in the testicle that needs to be surgically removed.
